D4: Identifying and analysing the root causes of the problem • Identify what caused the problem through brainstorming with the ProblemSolving Team and map each potential root cause. Find escape points for the problem. D5: Defining, choosing, and planning the best permanent actions • Determine with the Team what the solutions are that are most likely to remove the root cause of the problem and address these issues with the escape points. • Create a list of corrective actions to solve the problem and prevent similar issues from occurring again. Include a deadline for the implementation of these actions and define indicators to measure the effectiveness of these actions. It is your Action plan. D6: Verifying that the permanent actions are effective and do not create new problems • Implement the corrective actions according to the Action plan. • Monitor the results and employee feedback over a long period and take notes of any negative effects. Setting up controls to confirm whether the solution is working or more needs to be done. D7: Take preventive measures for future problems • Learn from the experience and modify your organisation’s process to monitor symptoms in a way to anticipate problems before they arise.
